Enrollment Instructions

We are excited to welcome your child to Bottineau Children’s Academy! To begin the enrollment process, we ask for some important information about you and your child. This information is required to meet state regulations and allows us to better care for and nurture your child.

To enrollment at Bottineau Children’s Academy:

Please complete all enrollment requirements, which include:

• Parent Sheet and Child Sheet
  These two forms must be filled out in full. You may either:
  • Print and deliver them to our location at 117 5th St W, Bottineau, ND 58318
  • Or complete them digitally, save as PDFs, and email them to bottineauchildrensacademy@gmail.com with the subject line: “Enrollment Files – Your Name”
